It doesn't looked Photoshopped, but she's holding onto him, and you can see him holding on to her. Also, the wall behind them could be his support for the photo.

Just an assumption.

I don't think its fake but I agree with j2tharome, between Cristina and the wall they are holding him up.

He looks very bad and VERY thin.

It looks doctored to me. His head doesnt square right with the center of his torso.
